Insurance

Level Up Therapy PLLC is not in-network with any insurance provider, but services may be covered if you have out-of-network benefits. We will provide all information needed for you to request reimbursement from your insurance company. 

Self Pay

Our 50-minute session rate is $150. All major credit cards and HSA/FSA cards are accepted as forms of payment.

Good Faith Estimate

Under the No Surprises Act, you have the right to receive a Good Faith Estimate for the total expected costs of services within a calendar year. Before beginning therapy, we will provide you with a Good Faith Estimate.
